On line patient services are now available for booking appointments, ordering repeat prescriptions and accessing your medical record on-line for patients aged 16 years or older ONLY. Appointments that are booked on-line can also be cancelled on-line. The Practice asks that as much notice as possible is given when cancelling on-line to allow another patient the chance to use that appointment slot.
You can sign up for Patient Access, or amend your current on-line services set up by completing the relevant form below and bringing it to reception along with your proof of ID. i.e. passport or driving licence.
Please note: Each patient access application form will require that individual to present with photographic identification. This is to ensure that staff have physically checked individual identification before allowing access to medical records.
Identification cannot be presented by a third party, it must be the individual. If you cannot get to any of the four sites to present this information, then we are unable to grant you access.

Patients can now download the MyGP app to book GP appointments and request repeat medication. Please remember that you can only book GP appointments using the app. However, appointments for Post-Natals and Medicals CANNOT be booked via the app due to the length of appointment that is required. To book these appointments with the GP, patients need to contact the practice.
Appointments for treatments that are usually carried out by our Nursing team, such as, immunisations & vaccinations, NHS Healthchecks, pill checks, diabetic, asthma and COPD reviews, should NOT be booked with a GP using the app.
If such appointments are booked, then you will be asked to cancel and re-book with the appropriate clinician. If in doubt, please contact the practice and speak to a receptionist who will be able to advise you.
Please be aware there is a known issue within the MyGP app that affects GP practices that have branch sites. As Highparks Medical Practice has four sites, currently the app will direct all patients to the main site at Cliffe Woods regardless of where your appointment is booked.
Patients are therefore asked to check the text reminder that is sent for the surgery site and to not rely on the directions given on the app.
